Acknowledgment of Risks

I understand that there are inherent risks associated with participation in play activities, including but not limited to:

Slips, trips, and falls
Collision with other participants
Injuries resulting from the use of playground equipment

I acknowledge and understand the risks associated with participation in play activities at The Grounds Indoor Play Space.

Waiver and Release of Liability

I, on behalf of myself and my child, do hereby release, waive, and discharge The Grounds, its owners, employees, and agents from any liability for any injuries or damages that may occur during participation in the Indoor Playground activities.

I agree to the Waiver and Release of Liability terms.

Medical Release

I authorize The Grounds to seek medical assistance for my child in the event of an emergency, and I agree to be responsible for any medical expenses incurred.

I authorize emergency medical treatment if needed and accept responsibility for any medical expenses.

Photography and Video Release

I grant The Grounds permission to take photographs and videos of my child during activities and to use them for promotional purposes.

I grant permission for photography and video recording of my child for promotional purposes.

Conduct Policy

I understand that my child must follow all safety rules and that I must remain on premises during play time.
